Home and Community Habilitation Services

Unique Aid designs Habilitation services to assist individuals to gain, maintain, and improve skills in the areas of self-care, daily living activities, social skills, and to enable the person’s involvement in community activities. These services help people live in their home; participate in local community activities; use community resources such as parks, pools, movies etc.; and access informal supports such as service clubs, senior programs, and other community opportunities.

Respite Services

Respite Services provision of short-term relief (respite) from the tasks associated with caregiving. Respite services encompass traditional home-based care, such as personal care, assisting in shower, feeding, and etc.

Respite can vary in time from part of a day to several weeks (24hour care).

It begins with your concern. Your worry about the health, safety and happiness of a family member in need is the real first step. By calling to Unique Aid and speaking to one of our staffing supervisors you are opening the door to a team dedicated to help. Our staffing supervisors will arrange a free consultation between your family and one of Unique Aid's experienced Registered Nurses.

Your free consultation and assessment takes place where it counts – in the home where care is needed. That's the best place for us to evaluate your needs and answer everyone's questions. The nurse will also ask questions to your family and loved one. Some of these will be routine questions about medication and diagnoses, but you may be surprised when the nurse asks “What do you like for breakfast?” or “Do you prefer a bath or shower?" These details are important to us; we use them to develop a complete care plan and make the best match between your family and one of our skilled caregivers.

When a caregiver is assigned to your family, they are assigned because their skill set matches the specific situation outlined in your unique care plan. Our goal is to match caregivers and patients who will both benefit in a long-lasting relationship. We provide regular and frequent supervision of the caregiver to assure quality of service and continuing satisfaction.
